Have you had a motor vehicle accident? Was anyone hurt? Was damage to all vehicles involved over $1,000 in value?
Minnesota State Statute 169.09 requires that every driver involved in a crash of $1,000 or more in property damage or resulting in injury or death must complete the Minnesota Motor Vehicle Accident Report Form and return the form to Driver and Vehicle Services within 10 days of the accident.
If you've been involved in an accident in the Orono Police jurisdiction, it is likely an officer responded and has written a report. The officer will attempt to obtain all necessary information about the accident and have it on file for you. However, the Minnesota Motor Vehicle Accident Report form needs to be completed by each driver. You may use the report of the officer to help you fill out this form.
You may obtain the Accident Report form from our Police Department or any police department that is convenient for you. You do NOT have to come specifically to Orono Police Department to obtain this form. All police departments have the basic form needed.
