There will be times an officer will ask you to provide information in writing about an incident you may have been involved in or witnessed. This form will ask for your name, date of birth, address and other related questions.
You should include as many details as possible about the incident in your own words. An officer can provide you with further instruction if necessary.
The statement form may be returned to the Orono Police
Department in-person at 2730 Kelley Parkway or by mail to Orono Police Department,
P.O. Box 86, Crystal Bay, MN, 55323.
When the statement is being returned, it is helpful if you know the name of the officer requesting the information and include that on the form.
